Operation Identity is a
non-profit organization which embraces all facets of the Adop-
tion Triad: adult adoptees, adoptive parents & birth parents. Our
tax-deductible annual membership dues are $20.00. If you want to support
Operation Identity, and receive our quarterly newsletter, please print the
Membership Application/Renewal Form below (by right-clicking on the form
and selecting “Print Picture”) and send the completed form, along
with your check or money order, payable to Operation Identity, to the addess
